Physical and Rehabilitation Medicine Doctor's Consultations
Physical and rehabilitation doctor consultation includes:
1. Collecting and evaluating information about the course of diseases, the way of life and work, and the surrounding environment.
2. Evaluation of medical documentation and conclusions prepared by another doctor's referral, medical specialist, functional specialist and diagnostic specialist.
3. When selecting persons to receive medical rehabilitation services, the doctor of physical and rehabilitation medicine assesses:
3.1. the person's state of health and related functional limitations;
3.2. potential for medical rehabilitation;
3.3. the motivation of the person and his belongings;
3.4. stability of health condition for receiving rehabilitation services;
3.5. the optimal way for a person to receive medical rehabilitation services, taking into account that medical rehabilitation services can be received in an outpatient medical facility, including a day hospital, an inpatient medical facility and at home.
4. In the case of chronic functional limitations, if rehabilitation is required for more than six months, a doctor of physical and rehabilitation medicine, implementing the dynamic monitoring of medical rehabilitation:
4.1. determines the intervals for monitoring the health status and functioning of the person, organizes the preparation or revision of the rehabilitation plan (at least once a year) and organizes its implementation;
4.2. prescribes the necessary medical rehabilitation technologies, medications, examinations and consultations;
4.3. sends the person to receive state-paid medical rehabilitation services;
4.4. if necessary, coordinates the developed treatment and rehabilitation plan with the family doctor and the social service of the municipality and coordinates its implementation.
5. The results achieved in the consultations are presented in writing in the IS of e-health, as well as issued on hand at the patient's request.
6. If the person in question needs medical rehabilitation services, he is issued a referral from a physical and rehabilitation medicine doctor and a rehabilitation plan for receiving medical rehabilitation, prepared in accordance with the procedure specified in the regulations of the Minister's Cabinet dated August 28, 2018 No. 555 "Organization and Payment Procedures for Health Care Services".
Physiotherapist's Consultations
Physiotherapist consultation includes:
1. collection and evaluation of information about the course of diseases, the way of life and work, and the surrounding environment.
2. evaluation of medical documentation and conclusions prepared by another doctor's referral, medical specialist, functional specialist and diagnostic specialist.
3. targeted evaluation of the patient's functional condition, using examination methods used in physiotherapy, analysis and interpretation of the obtained data;
4. preparation of a physiotherapeutic opinion and determination of the long-term and short-term therapeutic goals of therapy based on it;
5. planning and implementation of the rehabilitation program according to the physiotherapeutic assessment and therapeutic goals.
